Rehabilitation Counselor

Full Job Description:

The PRP counselor will be expected to provide individualized training for our severe and persistently mentally challenged adult and child clients as indicated in their Rehabilitation Plans. The successful candidate is expected to work under the Rehabilitation Coordinator to assist our clients to achieve their goals in the areas of self-care, social skills, and self-management skills, as well as to support them in relevant educational/vocational career pursuits. The ability to establish a professional relationship with referral sources to generate referral opportunities will be specially considered.

Essential Functions:
  • Review your clients’ charts weekly for relevant updates.
  • Provide PRP counseling to clients at home (offsite) and at the clinic (onsite)
  • Ensure that one of your patient’s counseling sessions is onsite except there is a waiver on record.
  • Address at least one goal from the patient’s rehabilitation plan at each counseling session. 
  • Turn in PRP contact notes weekly.
  • Turn in the remaining contact notes and timesheet latest by the 5th day of the subsequent month.
  • Ensure complete and proper documentation of your service.
  • Attend all scheduled meetings and training sessions.
  • Perform assignments delegated by the Rehab Coordinator or Program Director, which include, but are not limited to, reviewing IRP/Reviews with clients, and obtaining necessary signatures.
  • Meet with the Rehab Coordinator face to face at least weekly.

Rehabilitation Specialist

Full Job description:

The Rehabilitation Specialist will supervise staff by company policies & procedures and State & Federal laws. Responsibilities include, but are not limited to interviewing, hiring & training employees. The addressing of complaints & resolving problems. Maintaining quality improvement goals as well as addressing various concerns at scheduled staff meetings. Overseeing client delivery, ensuring performance measurements, & outcomes are met.

Essential Functions:
  • Completes Rehabilitation Assessments to identify the rehabilitative needs of participating consumers.
  • Lead and coordinate Individual Rehabilitation Program team meetings with engaged stakeholders, including therapists and consumers.
  • Develop Individual Rehabilitation Plans for each consumer, with domain, short and long-term goals, and interventions.
  • Shares/transmits the mission and vision of JK Health Systems, LLC to staff and clients, and families served.
  • Maintain a manageable caseload to ensure outcomes and practice requirements are met while taking into consideration the level of care of clients served.
  • Oversee the program and implementation of rehabilitation services
  • Employ and supervise appropriate staff to conduct rehabilitation services for clients participating in the program.
  • Provide orientation, supervision, and training including coordinating 60 hours of training to PRP Workers with the support of the staff developing coordinator, to assure the support and goal attainment of participating consumers. 
  • Collect monthly staff invoices, review invoices for accuracy, and sign invoices before submitting them to the director or billing.
  • Review and sign client’s daily contact notes/ monthly progress notes and record in client tracking record

Therapist/Clinician

JOB DESCRIPTION:

The Therapist/Clinician is responsible for the provision of comprehensive clinical social and case management services to the patients and their families. This includes the provision of individual, family, group therapies, crisis intervention, advocacy, assessment, planning, discharge planning, interdisciplinary teamwork, and community liaison within the limits of privileging. Therapists are responsible for formulating and implementing appropriate treatment strategies for patients and their families on their assigned caseload and for timely and satisfactory documentation of all treatment efforts. The Therapist/Clinician adheres to standards and regulations as set forth by the agency, the respective licensing board, and COMAR. The Therapist/Clinician is expected to abide by all safety rules and regulations of the department and facility. The Therapist/Clinician is responsible to carry out the mission, philosophy of care, and policies of JK Health Systems,LLC. Performs other duties as needed to achieve program goals and objectives.

SUPERVISION AND COORDINATION OF SERVICES

The Therapist/Clinician reports directly to their immediate supervisor and/or the Clinic Director. The Therapist/Clinician collaborates with all other disciplines through interdisciplinary teamwork and assigned responsibilities. The Therapist/Clinician adheres to the ethics of their respective profession.

ESSENTIALS FUNCTIONS:
  • Complete documentation in the EMR within 72 hours of completed sessions
  • Complete treatment plan developments before the 5th session and ensure signatures by all members of the client’s treatment team.
  • Coordinate referrals for additional levels of service as needed and requested by the family.
  • Address any clinical concerns with the direct supervisor.
  • Participate in weekly supervision.
  • Maintain clients and caseload over school breaks by providing community-based services.
  • Collaborate with service providers to ensure all areas of clients’ treatment needs are being addressed.
  • Utilize evidence-based therapeutic framework, counsel clients and patients, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
  • Guide clients in the development of skills and strategies for dealing with their problems using evidence-based modalities.
  • Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
  • Develop and implement treatment plans based on clinical experience and knowledge.
  • Modify treatment activities and approaches as needed to comply with changes in the client’s status.
  • Learn about new developments in their field by reading professional literature, attending courses and seminars, establishing and maintaining contact with other social service agencies.
  • Counsel family members to assist them in understanding, dealing with, and supporting clients.
  • Maintain confidentiality of records relating to the client’s treatment.
DESIRABLE MINIMUM QUALIFICATIONS

Knowledge, Skills, and Abilities:

  • Must have experience in working with and the ability to relate to the population to which they are credentialed to work, such as children, adolescents, adults, and/or geriatrics.
  • Possess knowledge of psychiatric diagnoses as defined by the American Psychiatric Association, assessment, human development, individual therapy, group therapy, and family therapy.
  • Demonstrate competency in psycho-social assessments, treatment planning, advocacy skills, and case management skills.
  • Demonstrate an understanding and sensitivity to serving a culturally diverse and/or special needs population while respecting patient confidentiality.
Education and Experience:
  • Mental Health Professional licensed by the state of Maryland.
  • Experience as required by the credentialing process.
